Welcome to Patient Services. These pages have been set up specifically for our patient's to find information and to request repeat prescriptions. Please note that for security reasons, none of your personal details are stored on our web server.

 

Order your Repeat Prescription

Repeat prescriptions can be ordered by using the tear-off slip attached to your prescription. These can be posted in to the surgery and, if a stamped addressed envelope is enclosed, we will post it back to you. Alternatively, requests for repeat medication can be handed in to the surgery. Due to the volume of requests we receive for repeat prescriptions we do not deal with repeat prescription requests over the telephone. Prescriptions will be ready for collection after 2 working days or can be forwarded to certain chemists for collection after 2 working days. Details of the chemist collection system are available from our receptionists.

To order your repeat prescriptions online, you can e-mail your prescription to the following e-mail address. prescriptions@maryfieldmedicalcentre.com. You can also order your repeat prescriptions online here.

It is important to keep your personal information up-to-date should the surgery or hospital need to contact you. Forms are available from reception or you can fill out an online here.

 

Physiotherapy Self Referral

You can refer yourself directly for physiotherapy without seeing your GP.

Surgery Closure
The surgery will be closed for the public holiday on Monday 27th May 2013. If you have an urgent medical problem when the surgery is closed, please contact NHS24 on 08454 24 24 24. The surgery will reopen at 8am on Tuesday 28th May.

Nurse Drop-in clinic
The the times for the nurse drop-in clinic are:
Monday - Friday: 08:30 to 10:30 and 11:00 to Noon.
This clinic is for Blood Tests and routine injections only. There is no need to book an appointment for this clinic.

MMR Catchup Campaign
Measles, Mumps and Rubella are highly infectious diseases that can leave children and adults suffering serious medical complications. We are currently running a catchup campaign for children aged 6 to 15 years and males aged 15 to 24 years who have not had 2 doses of the MMR vaccine and therefore not fully protected. We will shortly be contacting those patients who have not had 2 doses of the MMR vaccine. If you have not yet received your letter, you can contact the surgery on 462292 and make an appointment with the practice nurse.
